All applicants/visitors must read and be familiar with the Visiting Rules for Residents and Visitors and complete the online Visiting Application.
Children’s Visiting Application Requirements and Supporting Documents
- Complete the Visiting Application – Minor form
- A copy of the court order granting legal guardianship or Power of Attorney Delegating Limited Powers for Visitation
- A certified copy of the minor’s birth certificate
