Collect and provide to your investigator, if applicable:

1. Copy of your two most recent pay stubs (if you are employed)
2. Copy of certificates of training/educational accomplishments (such as school transcripts/certificates)
3. Physical/mental health records or letters from doctors/hospital
4. Copy of Certificates of Completion (such as completed treatment or classes)
5. Letters of character reference (from companions, family, employer, coworkers, etc.)
6. Military records, such as DD-214
7. Social Security confirmation letter
